How Gastric MALT Lymphoma is Treated

Fish are continuously exposed to a microbial-rich environment (freshwater or seawater) that circulates through and reaches every epithelial barrier of their body. The basal membrane of M cells is invaginated, forming a pocket that typically contains one or more lymphocytes (T-cells or B-cells) and occasionally macrophages (Gebert and Pabst, 1999; Pabst, 1987). The outlook is good, even when the lymphoma is quite widespread. Whether this latter group should be categorized as MALT lymphoma remains controversial; at least some of these lymphomas seem to be derived from the MALT.

Effector sites, on the other hand, are those where the effector cells after extravasation, retention, and differentiation perform their action, for instance by contributing to the formation of secretory IgA antibodies [ 16 ]. The recent development of an anti-trout CD8α antibody [ 32 ] has enabled a number of in-depth studies pertaining cytotoxic T cells in this species. In this issue of PLOS Biology, Lau et al. The concept of MALT was originally based on mouse repopulation experiments with B cells obtained from BALT and PPs as well as the surrounding LP. 1 , 88 However, although resemblance to centrocytes is the general rule, the morphologic pattern of the neoplastic cells can cover quite a large spectrum.

Immunostaining for CD21 (follicular dendritic cells), Ki-67, Bcl-2, and Bcl-6 may help identify residual reactive follicles; immunostaining for those proteins as well as for CD10 can also aid in distinguishing follicular colonization from the very rare extranodal follicular lymphomas. Although mononuclear cells immunoreactive for anti-chlamydia antibodies were not found within blood vessel lumina in the ocular adnexal lymphoma samples studied, available immunohistoIHCchemical data do not allow the distinction between these cells being resident or circulating elements. Each of 13 new isolates tested had a unique complement of genes. 32 , 39 The presence of the B-cell clone that would become predominant in the transformation to MALT lymphoma has been demonstrated in H pylori gastritis specimens taken several years before development of the lymphoma. The MALT is covered by a subset of differentiated microfold (M) cells, ECs, but not goblet cells, and underlying lymphoid cells that play central roles in the initiation of mucosal immune responses. Some grow very slowly, whilst others grow quickly and need aggressive treatment. Paneth cells in crypt regions of the GI tract produce anti-microbial peptides (AMPs), or α-defensins, while ECs produce β-defensins [10],[11] for host protection (Figure 1).

MALT is a crucial component of a functional immune system because mucosal surfaces, such as the nasal passages, are the first tissues onto which inhaled or ingested pathogens are deposited.

Staging and grading of MALT lymphoma

An alternative hypothesis is that VacA acts directly on T cells by inhibiting activation of intracellular signaling pathways. Partial involvement of the right pyriform sinus was again noted. Lymph nodes are usually bean-shaped, with an indented region known as the hilum. How different subsets of mucosal T cells respond to different antigens and how the microbiota contributes to mucosal T cell education is not known. Similar to findings observed in gastric MZLs , in which eradication was followed by an objective response in 70–80% of case patients, 2 out of 4 of case patients with ocular adnexal lymphoma and measurable disease showed objective regression after antibiotic treatment.

Flow cytometry was used to identify transfected cells which express pAPN by evaluating the binding of polyclonal rabbit pAPN-specific antibodies to the cells. In principle, the lack of differentiated inductive and effector mucosal sites in fish may permit a more simple mechanism of function for teleost MALT. First the adhesion of the pAPN-specific antibodies to pig enterocytes was demonstrated in vitro by using a pAPN-transfected cell line (BHK-pAPN) as well as in vivo using intestinal loops. However, MALT usually designates the areas where immunopoiesis and immune reactions occur, and thus the secondary lymph organs and the diffuse effector cells located along the mucosae.

Clinical Features

The high prevalence of CD8T cells in the gills is remarkable and unique to teleost respiratory organs compared to mammalian lungs, where this cell subset is present at low abundances.

For example, a paucity of murine models exist to study food or milk allergies that usually affect the duodenum of the small intestine [30]. Similarly, in teleost fish, both IgT and IgM are detectable at the protein level in a number of mucosal secretions using immunoblotting or ELISA (for a summary see [ 17 ]). Loss of either T or B lymphocytes also led to increased EC apoptosis, suggesting that both cell types are required to protect the epithelium from cell death.

Host Defenses At Mucosal Surfaces

Mantle cell lymphomas and B cell chronic lymphocytic leukemias (B-CLLs) expressed clearly restricted albeit different CDR3 repertoires. Thus, it is possible that with the development of antibodies to detect CD11a, teleost TRMs may be described in the future. In the above examples of lymphoma pathogenesis, defective immunoregulation characterized by either excess lymphocyte activation and proliferation or defective apoptosis leads ultimately to clonal expansion and lymphoma. 130 However, eradication of H pylorishould be attempted (in addition to chemotherapy) even in the setting of large-cell lymphoma, because this approach may eliminate a residual or relapsed low-grade component that can be responsible for tumor recurrence following antigen stimulation.

Clinical features : Although MALT lymphomas arising in different organ systems share many pathologic and clinical similarities, there are also important differences. Autoimmune hemolytic anemia in dogs, consequently, pharmacological or dietary modification of cellular iron trafficking enhances host resistance to intracellular pathogens but may increase susceptibility to microbes in the extracellular compartment and vice versa. These low grade (indolent) lymphomas are most often diagnosed in the stomach (called gastric MALT). A mucosa is composed of an epithelial lining and a lamina propria rich in blood and lymph vessels; sometimes glands and smooth muscles are also found. Gastric infection, even if common among OAL patients, does not influence clinical presentation. Surgery for non-gastrointestinal MALT lymphoma is usually restricted to excisional biopsies. These follicles, having true germinal centers, are similar to the follicles of the spleen and lymph nodes. If this is the case, then the parallelism between the association of gastric lymphomas with –related chronic gastritis and the potential pathogenic link between chronic conjunctivitis related to chlamydial infection and ocular adnexal lymphomas appears to be worthy of analysis. Previously, hydroxyurea (HU) has been used to distinguish between HU-sensitive (plasmablast) and HU-insensitive (plasma cell) activities in rainbow trout [ 28 ]. Mucosal vaccination also has other advantages over injected vaccines, particularly the ease of administration to large numbers of fish of any size. This is not an indication of a security issue such as a virus or attack.

Depending on their localization in the body, MALT receive specific names. There are characteristic histological features. For cases of MALT lymphoma not caused by H. The mucosa of the digestive, respiratory and urinary tracts often contains small aggregations of lymphocytes called lymphoid follicles. The outlook for people with gastric MALT lymphoma is usually good with about 80% of people surviving beyond the 5 year milestone and 77% going on to have disease free survival at 10 years. These vessels pierce through the capsule and drain into the space below, known as the subcapsular sinus.

In the conjunctiva, there are many secretory IgA lymphocytes. We currently have no indication that these cells exist in teleosts.
